Two clear pages in passport and photocopy of information page in passport
Two recent passport photographs
One signed and completed application form
Copy of invitation letter from inviting company containing passport details and purpose of trip
For multiple entry an invitation letter from the Immigration Department of the Ministry of Internal Affairs of Azerbaijan is required
Original UK company letter containing passport details and purpose of trip
For express lodging a telex of authorisation is required
