Zanzibar is part of Tanzania and therefore the Tanzania visa covers both destinations. There are two option for Tanzania visas. If you are entering Tanzania just once during your visit then apply for the single entry visa. If you need to enter Tanzania more than once, then you will need a multiple entry visa however you must show proof that you will be entering Tanzania more than once.
